Documentation may include:
- Medical Verification of Disability - If you don't have documentation in written form, you may take the verification of your disability form to a licensed professional
- Psychological Verification of Disability - If you don't have documentation in written form, you may take the verification of your disability form to a licensed professional
- Audiogram (hearing loss verification)
- Most recent Psycho Educational Assessment Report (may include IEP or 504 Plan but not required).
- Visual Acuity
Students are advised to register for classes on their registration date.
